Dear parents/caregivers,

Parents/caregivers who have children registered for Remote Learning for the 2022-23 school year must complete an attestation form. The Ministry of Education has directed boards to review in-person proof of residence within the province as part of this process.

Parents/Guardians can provide this at the home school office. Please call or email to book an appointment. As per Ministry requirements, documentation is to be submitted in person by the parent or guardian.

The following documents meet the Ministry requirements:

  • Rental/Purchase agreement

  • Current utility/phone bill (issued within the past two months)l

  • Current property insurance bill (issued within the past two months)

  • Current tax bill (issued in the past quarter)

  • Government document (i.e. Service Ontario, CRA – dated within the past two months)

  • Bank statement (issued within the past two months)
